Join Mark and Charles as they explore the shadows of military history, uncovering the stark reality of psychiatric neglect in the armed forces. In this episode your hosts explore Chapter 3 of their book, "Cruel and Inhumane Handling," shedding light on the military's failure to address the full cost of war on soldiers' mental health.

Hear the chilling accounts of ancient and historical practices of punishment for those deemed weak or cowardly, from the Roman legions to the American Civil War. Discover the troubling legacy of branding, barreling, and flogging, and how these brutal tactics aimed to maintain discipline by making examples of those suffering from psychiatric injuries. As Mark and Charles reveal, these "dark strategies" are not relics of the past but continue to haunt military mental health care in various forms, perpetuating a cycle of trauma and neglect.

Through their conversation, the hosts expose the complexities of distinguishing cowardice from war stress injuries and the military's dilemma of balancing the need to fight wars with the obligation to protect the mental well-being of its soldiers. They discuss the phenomenon of "Old Sergeant Syndrome," the inevitable breaking point of even the most battle-hardened veterans, and the implications for military discipline and cohesion.

This episode is a must-listen for anyone interested in the intersection of military history, mental health, and the human cost of war. Tune in for a sobering yet necessary discussion on the urgent need for change in how we address the invisible wounds of battle.

War wounds are not always visible, and the battle scars of the mind can be as debilitating as any physical injury. Mark and Charles open up a long-overdue discussion on the unseen struggles of military mental health, tracing the historical neglect and systemic shortcomings in armed forces' approaches. The conversation is both a history lesson and a current affairs class, as we uncover declassified documents and share from-the-front anecdotes that reveal the stark contrast between lessons learned from physical warfare and the often-ignored psychological aftereffects. Mark and Charles shed light on the depth of these issues, from the repeated cycle of learning and forgetting to the pervasive stigma that keeps our service members from seeking the help they so desperately need.

This conversation is not just about the failures of the past but also about the potential for reform—ensuring that every soldier's sacrifice is honored, not just in medals and commendations, but in the care they receive for all their wounds, seen and unseen.

The psychological toll of war is vast, and the social costs of war’s psychiatric casualties extend even further. Yet military mental health care suffers from extensive waiting lists, organizational scandals, spikes in veteran suicide, narcotic over prescription, shortages of mental health professionals, and inadequate treatment. The prevalence of conditions such as post–traumatic stress disorder is often underestimated, and there remains entrenched stigma and fear of being diagnosed. Even more alarming is how the military dismisses or conceals the significance and extent of the mental health crisis.

The trauma experts Mark C. Russell and Charles Figley offer an impassioned and meticulous critique of the systemic failures in military mental health care in the United States. They examine the persistent disconnect between war culture, which valorizes an appearance of strength and seeks to purge weakness, and the science and treatment of trauma. Instead of reckoning with the mental health crisis, the military has neglected the needs of service members. It has discharged, prosecuted, and incarcerated a large number of people struggling with the psychological realities of war, and it has inflicted humiliation, ridicule, and shame on many more.

Through a far-reaching historical account, Russell and Figley will detail throughout their episodes how the military has perpetuated a self-inflicted crisis. And will offer actionable prescriptions for change and a comprehensive approach to significantly improving military mental health.

Mark, Charles and Kat confront one of the most pressing and persistent issues facing our armed forces: the chronic neglect of military mental health care.

In this second to last episode of this series, Mark prepares to step away from the podcast, he reflects on his personal journey—one that began with the invasion of Iraq in 2003 and has been defined by a steadfast commitment to advocating for mental health reform. Together, the hosts take a hard look at the barriers to change, from entrenched stigma to slow-moving policy, and the immense toll this neglect has taken on service members and their families.

The conversation navigates:

The systemic challenges in military mental health policy

Personal stories and professional insights from decades of advocacy and research

The cultural and institutional stigma that still haunts mental health treatment

Bold ideas for legislative and structural reform

This heartfelt discussion is both a tribute to those affected and a rallying cry for those ready to create change. Join us for a powerful episode that reminds us all: the invisible wounds of war deserve to be seen—and healed.
